Overview

Your job is to be the hub of the patients multidisciplinary team, advocating, coordinating, educating, supporting, and counseling along the care journey. As a Patient Navigator Nurse, you are a clinical resource with expertise in specialty care management, connecting and liaising with patients recovering from strokes, cancer, and long-term illnesses. You remain positive, upbeat, and dedicated to assuring quality and achieving the best possible outcomes for patients.

Your Everyday

• Coordinate written and verbal communication relating to the medical treatment plan between the patient, family, physician, physician staff, and other team members.
• Communicate and coordinate the treatment plan to internal and external departments and agencies, establishing and maintaining the continuity of patient care.
• Assist patients in understanding their diagnosis, treatment options, and the resources available.
• Review medical documentation to assist physicians in obtaining complete and accurate documentation of the medical treatment plan.
• Serve as an essential link between patients and all other care providers.
• Follow patients through the care continuum, collaborating with care management resources and facilitating appointments for consults and support services including social workers, PT, OT, ST, dietitians, physicians, and financial resources.
• Collect data, track outcomes, and support strategic planning processes; provide nursing care and guidance from screening to survivorship by planning, implementing, and evaluating the patients care.
• Assist patients and families in finding solutions, addressing treatment-related problems, and providing education to facilitate seamless care and support difficult decisions.
• Review policies related to services and update as appropriate to enhance professional practice.
• Serve as a resource for community educational events and staff education (e.g., health fairs, screenings, symposiums, lectures).
