Position : Admissions Coordinator

Shift : Full Time

Salary : $26–$28 per hour

Join a dynamic, compassionate team dedicated to making a real difference in the lives of our residents. At Azure Shores Rehabilitation, we’re looking for an Admissions Coordinator who is passionate about patient care, relationship-building, and driving positive first impressions of our facility.

Key Responsibilities :

Monitor and identify operational areas affecting admissions, report findings to administration and the corporate team, and assist in developing systems to resolve issues.

Support marketing outreach by attending functions and events to promote the facility’s positive image, build relationships, and increase referral and admissions volume.

Explain visitor and guest privileges, restrictions, and resident care procedures.

Coordinate with hospitals to set up admissions and prepare all necessary arrangements.

Establish and maintain relationships with local hospitals, physicians, and community organizations to generate consistent admissions and maintain a positive facility reputation.

Ensure financial verification is accurate and complete before patient admission, and communicate this information to the interdisciplinary team.

Maintain knowledge of Medicare / Medicaid / Managed Care reimbursement processes.

Complete and report monthly statistics to your team.

Requirements :

Payor Types, Medicare, and Insurance

Minimum of 1 year of experience in Admissions within a Skilled Nursing Facility

Knowledge of Medicare requirements and insurance providers (e.g., PDPM, HMO rules, RUGS II & III, Medicare prescription plans)

Familiarity with EMR software systems

Understanding of CMS guidelines, DOH regulations, and various insurance plans

Strong verbal and written communication skills
