Bayside Health & Rehabilitation Center in Virginia Beach, Virginia is looking for an experienced Director of Discharge Planning and Social Services to help residents and families prepare for their transition home. You will directly coordinate the patient's discharge including supplies, transportation, and other services to ensure a safe transition. If you are passionate about helping people and delight in their recovery, we want to talk to you!
We are looking for an energetic and dynamic self-starter with 2 or more years of experience in Discharge Planning and Social Services in a healthcare setting. The ideal candidate has a history of successful positions involving direct customer service, social services, coordination, and problem-solving, and teamwork. Strong communication and collaboration skills are a must.
Your day-to-day
Qualified candidates will have a 4-year degree in Social Work, Psychology, Sociology, Education, Counseling, Criminal Justice, or other human services related degree and at least 1 year of Social Work experience in a healthcare setting.
