Key Responsibilities of the Physical Therapist
- Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals
- Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for current diagnosis, past history, and precautions
- Establish measurable goals and develop a treatment plan
- Regularly re-assess treatment plans and patient progress
- Communicate patient needs and progress effectively
- Consult with medical providers regarding patient care
- Recommend and assist in securing adaptive equipment for patients
- Maintain and submit documentation as required
- Supervise Physical Therapist Assistants and Aides in patient-related activities
- Participate in staff development and continuing education
Requirements of the Physical Therapist
- Current Physical Therapist license
- Experience in home health required
- HCHB experience preferred but not required
