About the Role:
We're seeking a compassionate and detail-oriented MDS Coordinator RN (LPN) to join our team in New London, Connecticut. As a key member of our interdisciplinary team, you'll be responsible for coordinating the care planning process for our residents, ensuring that their needs are met and that their goals are achieved. You'll work closely with our nursing staff, therapists, and other healthcare professionals to develop and implement individualized care plans that promote optimal health and well-being.
Responsibilities:
Coordinate the MDS process, including assessing resident needs, developing care plans, and conducting quarterly care plan updates
Collaborate with the interdisciplinary team to identify resident needs and develop strategies to meet those needs
Ensure that resident care plans are up-to-date and reflect current resident needs and goals
Monitor and report on resident progress, identifying areas for improvement and implementing changes as needed
Maintain accurate and detailed records of resident care and progress

Requirements:
Current LPN licensure in the state of Connecticut
Experience in a post-acute care setting, preferably in a MDS Coordinator role
Demonstrated ability to work effectively in a fast-paced environment
Excellent communication and interpersonal skills
