Position Summary :
Plan, implement and evaluate patient care programs designed to increase, maintain or restore function, minimize or prevent disability, and assist patients in reaching their maximum performance levels.
Essential Duties and Responsibilities include the following :
- Reviewing and evaluating patients' conditions and medical records, performing tests or measurements, interpreting findings, and maintaining documentation.
- Conducts discipline-specific patient evaluations, assessments, consultations, and treatment, as a member of an interdisciplinary team, in accordance with program protocol and federal and state regulations and guidelines
- Helping patients attain maximum muscle strength and motor skills as well as accepting and adjusting to the limiting effects of their disabilities.
- Carry out individually designed programs of physical treatment to maintain, improve or restore physical functioning, alleviate pain and prevent physical dysfunction in patients
- Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
- Confer with the patient, medical practitioners and appropriate others to plan, implement and assess the intervention program.
- Direct and supervise supportive personnel, assessing their competence, delegating specific tasks to them and establishing channels of communication.
- Develop and update appropriate plan of care in collaboration with the patient / family and related to the person's age and lifestyle with measurable long and short term goals.
- Follow intervention protocols and alter plan of care appropriately
- Work intimately with the director of Physical Therapy to improve function and flow of patient schedules for all therapy teams
- Performs miscellaneous job-related duties as assigned by director of PT
- Performs all other duties as directed by management.
The pay range for this position at commencement of employment is expected to be between$110k - $130k annually; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.
The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and / or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

If hired, employee will be in an "at-will position" and the Company reserves the right to modify base salary (as well as any other discretionary payment or compensation program) at any time, including for reasons related to individual performance, Company or individual department / team performance, and market factors.

Requirements
Education and / or Experience :
Must possess valid California PT licenseCPR certificationTwo years of PT experience in outpatient or inpatient setting preferredNew Grads are welcomeAbility to effectively communicate medical information, test results, diagnoses and / or proposed treatment in a manner easily understood by the client.Knowledge of anatomy, physiology, and / or kinesiologyAbility to gather and analyze statistical data and generate reports.Ability to safely lift, and physically manipulate patients.Knowledge of the administration, indications and contra-indications of various physical agents and physical therapy techniques.Knowledge of clinical operations and procedures.Ability to instruct physical therapy patients in the use of facilities and equipment.Skill in performing a range of manual physical therapy techniques for patients.Knowledge of the operation and maintenance of therapy facilities and equipment.Ability to monitor and / or maintain quality control standards.Ability to observe, assess, and record symptoms, reactions, and progress.Ability to interpret, adapt, and apply guidelines and procedures.Ability to plan, develop and implement physical therapy programs.Ability to perform diagnostic tests and evaluations for physical therapy patients.Ability to communicate effectively, both orally and in writing
