Our Program of All-Inclusive Care for the Elderly (PACE) center in the Greater Denver Region is looking for a compassionate, skilled Advanced Practice Provider (NP or PA) to join our collaborative Interdisciplinary Team (IDT).

- Low Volume, High Impact: Manage a mature panel of 75-80 seniors with a gradual ramp-up.

- Mon-Wed (On-Site): Clinic visits, acute assessments, daily IDT meetings.
- Thu (Community): Local nursing facility / SNF / home visits.
- Fri (Remote): Work-from-home charting, care coordination, follow-ups.
- Light On-Call: Dedicated Nurse Triage handles first-line calls; provider escalation is rare and receives supplemental call pay.
- Autonomy & Support: Direct reporting to the Medical Director with zero RVU quotas.
Requirements
- Active (or eligible for expedited) Colorado NP or PA license & DEA certification.
- AGPCNP, FNP, ACNP, or NCCPA certification.
- 2+ years experience in geriatrics, internal medicine, family practice, SNF, or home care preferred (standout new grads with strong RN/rotation background considered).
