What You’ll Do:
- Provide compassionate hospice care to patients in a residential or care facility setting
- Conduct up to 4 patient visits per day, focusing on symptom management and palliative support
- Collaborate with interdisciplinary teams to develop personalized care plans
- Document patient interactions and care updates accurately
- Educate families and caregivers on patient needs and comfort strategies
- Ensure compliance with all hospice regulations and standards
What You Need:
Required:
- Valid Nurse Practitioner license in Texas
- Experience in hospice or related healthcare setting
- Strong interpersonal and communication skills
- Ability to manage a flexible schedule and deliver compassionate care
Preferred:
- Previous experience in palliative or end-of-life care
- Certification in hospice or palliative care
