Neighborhood clinic serves its local, over 65 population. Value-based care model structured to treat the whole person & reduce unnecessary hospital admissions. Organization is one of the fastest growing geriatric care providers nationwide, operating 300+ centers across multiple states.

ROLE
\n
Will see patients in 100% outpatient clinic with No Nights, No Weekends!
\n
- \n
- Clinic Hours: Monday-Friday 8am-5pm with minimal call \n
- Will treat complex conditions & analyze to determine best course of treatment \n
- Foster relationships with each patient, spending more time, not less \n
- Will supervise /collaborate with Advanced Practitioner Nurses or Phys. Assistants \n
- Multidisciplinary team (nurses, behavioral health specialists & pharmacists \n

PACKAGE
\n
Salary range commensurate with experience $220-$250K
\n
Annual bonus based on quality metrics + Sign on bonus!
\n
Potential for over $300k total income year 1, promotable to Lead
\n
