RESPONSIBILITIES

Assists with on-going assessments, development, and implementation of individual support plans; provides active treatment, quality care, support, and supervision, assists individuals with self-help, task learning, and community utilization skills based on individual’s support plan; maintains individual records and documentation according to all applicable standards; assists with planning and implements community and center-based activities to meet individual needs, monitors and maintains the safety and wellness of individuals, communicates with other community agencies and programs; and provides transportation to implement the individual’s Individual Support Plan. Hours of the program may include weekdays, weekends, and evenings.

REQUIREMENTS

Minimum Requirements

M1 : High school diploma or equivalent

M2 : Valid driver’s license and safe driving record

Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request. Our Agency maintains a drug-free workplace.

ANNUAL SALARY RANGE

$32,493 - $56,863

3% Increase for Certified Nursing Assistant
