We are seeking a compassionate and detail-oriented  Medicare Care Coordinator  to join our UM Support Services team. This role is pivotal in ensuring a smooth and supportive experience for our Medicare patients—from enrollment through ongoing care. The ideal candidate will be a proactive communicator, skilled in care coordination, and passionate about improving patient outcomes.

Key Responsibilities :

• Welcome and educate new Medicare patients on benefits, care options, and pharmacy coverage.
• Handle inbound calls regarding benefits, appointments, medications, and care coordination.
• Schedule initial and follow-up appointments with Primary Care and Specialist Providers.
• Verify patient eligibility and manage ongoing care coordination.
• Conduct outbound outreach to promote preventive care, medication adherence, and wellness visits.
• Guide patients through pharmacy formularies and coverage options.
• Use EZ-Cap, Cozeva, and Salesforce to manage patient data and document interactions.
• Ensure timely and accurate documentation in compliance with reporting standards.
• Connect patients with health plan brokers for benefit guidance.
• Verify authorization status prior to service delivery.
• Exhibit empathy and professionalism while resolving escalated calls.
• Maintain regular attendance and participate in occasional travel (up to 5%).
Qualifications :
• Required :

High School Diploma or GED

Associate degree in a related field

This is a Contract position based out of Sacramento, CA.

Pay and Benefits

The pay range for this position is $21.00 - $26.00 / hr.
