Join our team as a Street Outreach Liaison , where you'll lead Street Outreach Advocates and partner across agencies to connect individuals with life-changing CALAIM services—including Enhanced Care Management, Community Supports, Behavioral Health, Street Medicine, and CHW programs.

- ? What You'll Do
- ?? Integrate Programs & Build Better Workflows
- Partner with ECM care coordinators, BH navigators, CHWs, and street medicine clinicians.
- Create smooth pathways from screening ? enrollment ? clinical follow-up.
- ?? Build & Strengthen Community Partnerships
- Engage food banks, shelters, and community-based orgs to create warm referral pathways.
- Develop shared calendars, co-host outreach events, and set up efficient data-sharing practices.
- ??? Lead Agency Collaboration
- Represent our program with city, county, state, and federal partners.
- Support cross-agency work with public health, housing authorities, and SAMHSA partners.
- ?? Train & Support Outreach Teams
- Deliver training on trauma-informed practice, documentation, and CALAIM eligibility.
- Ensure proficiency with mobile tools, CRM systems, and field-based workflows.
- ?? Use Data to Drive Impact
- Track KPIs like daily contacts, enrollment conversions, and referral success.
- Identify trends, adjust outreach strategies, and share insights with leadership.

- Hosting a roundtable with local shelters and nonprofits to expand warm referrals.
- Co-writing a grant proposal with city partners to fund housing or outreach initiatives.
- Running joint enrollment workshops at a women's shelter.
- Piloting a co-location partnership at a food bank to boost Community Supports applications.
- Standardizing outreach trainings across multiple sites and improving data accuracy.
- ?? What Makes You a Great Fit
- 2–3 years of experience in ECM outreach, community engagement, or outbound sales
- Strong relationship-builder with experience forming partnerships
- Confident communicator—comfortable presenting to partners and agencies
- Data-driven and CRM-savvy
- Culturally humble and skilled at engaging diverse, underserved communities
- Valid driver's license + willingness to work in community settings (field-based)
