Hello, Compassionate Community Care (CCC) is a prominent San Francisco-based Home Care agency. We are NOT a traditional Home Health company.
Rather, we offer customized in-home consulting & planning, and ongoing in-home caregiver services to primarily adult / geriatric individuals who need help adapting their home to comfortably, sustainably function at home.
The ongoing in-home care is provided by our non-nursing caregivers.
CCC is looking for a full-time Registered Nurse or LPN / LVN to help market our services and provide in-home consulting services that include creating, implementing, and overseeing client care plans.
We need an individual with lots of energy and a great attitude, to help clients feel safe & comfortable with proposed care plans.
The most successful candidate must be happy with marketing to potential referral sources AND to clients!
The vast majority of your day-to-day will be in San Francisco, so you must have reliable transportation that is capable of regularly navigating around The City.
This is a not a patient-treating job opportunity, so it is well-suited for a nurse looking for a break from direct patient care.
A few details about the job :
- Thoughtfully assess clients' homes for safety, then create and implement a customized care plan that prospectively includes professional installation of adaptive equipment, crisis / emergency planning, and ongoing in-home caregiver visits
- Guide clients through the intake process
- Collaborate with professionals and home care teams to ensure proper plan implementation.
- Establish and maintain relationships with client referral sources to help grow CCC
- Identify, organize, and attend educational events as part of CCC marketing efforts
- Participate in a weekends / holidays on-call rotation, assisting clients during off-hours as needed
- Assist with training new caregivers on Hoyer Lift, gait belt, performing ADL's, etc.
- Act as client advocate with vendors, medical personnel, and families
Preferred Qualifications :
- RN or LVN / LPN, current or formerly licensed in the State of California
- Preferred bachelor’s degree in Gerontology or a relevant field
- Demonstrable in-home geriatric care experience
- Prior caregiver training experience preferred
- Excellent interpersonal and communication skills
- Strong computer skills, proficient with social media and Microsoft Word, Excel, Outlook
- Excellent problem-solving skills
- Comfortable working with diverse cultural backgrounds
- Strong time management skills,
- Excellent multi-tasker and self-starter
- Proven successful marketing / outreach experience
