Gastroenterology Physician

ALL OF THE FOLLOWING ARE REQUIRED AT NAME CLEAR UNLESS NOTED OTHERWISE.

Board Eligible (must become Board Certified within a year) or Board Certified in Gastroenterology - REQUIRED

• MI license (or willing to obtain) - REQUIRED
• Fellowship-trained in Gastroenterology - REQUIRED
• BLS (or willing to obtain) - REQUIRED
• Clean Malpractice and License History - REQUIRED
• Current COVID and flu vaccinations (note in 'Additional Submission Details' field) - REQUIRED
• 5-8 years of GI work experience (note in 'Experience' field) - PREFERRED
• ERCP certification (will also consider someone that needs additional proctoring) - (note level of training in 'Additional Certification Details' field) - REQUIRED
• ITIL or other industry certification (note any certifications held in 'Additional Submissions Details' field) - PREFERRED
• PMP certification (note in 'Additional Submission Details') - PREFERRED
• Does the provider require visa support - J1 or H1B? (note in 'Additional Submission Details' field) - REQUIRED
