About the Role : Crisis Response Team : Behavioral Health Worker
As a member of the Crisis Response Team, you will play a crucial role in providing high-quality care and assessments to individuals and families in need of crisis intervention. This specialized team manages emergency walk-in services, crisis calls within the Weld County area, and after-hours emergency on-call services across a 12-county region in Northeast Colorado.
The Crisis Response Team conducts thorough assessments to understand each client's needs and circumstances, helping determine the most appropriate level of care in the least restrictive setting. Additionally, the team provides intensive care coordination for Weld County clients and discharge planning for individuals transitioning from higher levels of care back into their communities.
Behavioral Health Workers support the implementation of individualized care plans for clients facing mental health and / or substance use challenges. They adhere to the policies, rules, regulations, and directives of North Range Behavioral Health while facilitating client engagement. Behavioral Health Workers act as sources of emotional support, assist with resource navigation and problem-solving, and advocate for clients' needs to ensure their active participation in care plans.
