Schedule : Availability to work a minimum of two 12-hour shifts every 4 weeks.

Under the direct supervision of a Registered Nurse, the LPN (Licensed Practical Nurse) provides direct nursing care to patients in the IPU in accordance with the Nurse Practice Act and / or Hospice Standards of Practice.

What are the essential duties day to day?

• Duties are delegated by and performed under the supervision of a registered nurse.
• Responsible for assessing patient / family needs and for implementing and evaluating the plan of care.
• Provides direct patient care according to the plan of care and in accordance with the Nurse Practice Act and Hospice Standards of Care.
• Provides nursing services (treatment of symptoms, preventative measures, etc.) according to the plan of care.
• Instructs family on IPU procedures as needed.
• Serve as an active member of the IDG and attend group meetings regularly.
• Continually re-assesses patient needs, notify physician, and under the direction of RN, modify plan of care as needed.
• Assists with chart audits to insure compliance with regulations.
• Actively participates in team and staff orientation.
• May pronounce death.
• Acts in a professional manner to collaborate with physicians, other health care providers, and interdisciplinary team members and makes timely reports of pertinent information to appropriate health care professionals.
• Documents data collected for assessments and interventions in an accurate and timely manner according to the nursing standards of this agency.
• Ability to lift, turn and transfer patients; minimum lifting requirement 40 pounds.
