Qualifications :
Education : Master's Degree in Social Work (MSW)Licensure : Active NY State licenseExperience : Minimum of 1 year of experienceTechnical Skills : Proficiency with electronic health records (EHR).Soft Skills : Empathy, cultural competence, and a patient-centered approach.Key Responsibilities :
Conduct comprehensive psychosocial assessments and diagnoses for patients and their families.Provide evidence-based individual, group, and family therapy.Develop, implement, and monitor individualized treatment plans in collaboration with a multidisciplinary healthcare team.Perform crisis intervention and risk assessments, implementing appropriate safety plans as needed.Advocate for patients and families to ensure access to community resources, social services, and financial assistance programs.Provide care coordination, including patient education, discharge planning, and referrals to appropriate follow-up care.Maintain timely, accurate, and confidential patient records in compliance with all state, federal, and organizational policies.Participate in case conferences, team meetings, and ongoing professional development opportunities.How
