About the Role :

Seeking a compassionate and skilled Licensed Master Social Worker (LMSW) to join our team in New York, NY. This hybrid role requires onsite presence every Thursday and offers a dynamic work environment supporting individuals living with HIV, transgender populations, and homeless members. You will provide telephonic and face-to-face care management services, ensuring members receive comprehensive, coordinated, and high-quality care aligned with their health goals.

Key Responsibilities :

Develop and implement individualized Plans of Care, collaborating with members, families, providers, and community resources.

Coordinate access to comprehensive healthcare services, including health coaching on HIV and chronic illnesses such as hepatitis C, diabetes, and hypertension.

Facilitate communication among primary care providers, case managers, and other healthcare professionals to promote coordinated care.

Monitor member health data and assess service utilization trends to anticipate and address member needs.

Document all care coordination activities in compliance with professional and regulatory standards.

Mentor and support HIV Services Health & Wellness Advisors, contributing to their professional development.

Participate in case conferences, quality improvement projects, and special departmental initiatives.

Ensure all care coordination adheres to Federal, State, and City regulations, aligning with the organization’s mission and values.

Qualifications :

• Licensed Master Social Worker (LMSW) credential required.
• Experience in HIV care, chronic disease management, or related social work preferred.
• Strong communication and interpersonal skills with the ability to work collaboratively.
• Proficiency with electronic health records (EHR) and care management documentation.
• Commitment to working with diverse and underserved populations.
