Job Summary:
Astoria Healthcare Center is seeking a dedicated and compassionate Occupational Therapist (OT) to join our in-house therapy team! As a Full-Time OT, you will have the opportunity to work with a diverse range of patients in our Skilled Nursing Facility and Rehabilitation center. Our Rehab Department has an excellent reputation in the community for providing high-quality rehabilitation services, and we are committed to providing quality patient care with responsiveness, courtesy, and gentleness.
Responsibilities:
Evaluate patients in a timely manner following physician referral
Develop effective treatment plans and provide education for clients, family, and caregivers as a subject matter expert
Communicate with supervisors and other healthcare team members regarding patient progress, problems, and plans
Participate in in-service training programs for other staff in the facility
Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy
Provide accurate and timely billing records and supporting clinical documentation
Instruct patient's families or nursing staff in maintenance programs
Coordinate necessary durable medical equipment and adaptive equipment for patients to facilitate safety and independence in ADL's
Ensure resident abuse or neglect does not occur
Observe confidentiality and safeguards for all patient information
