Support Whole-Person Care as a Nurse Care Manager in Our Behavioral Health Healthcare Home
We’re seeking a Nurse Care Manager to join our integrated care team and make a meaningful impact on the health of individuals with behavioral health needs. In this role, you’ll coordinate physical health support for clients in our Healthcare Home program—bridging the gap between behavioral and medical care to help clients live healthier, fuller lives.

Key Responsibilities
Coordinate care for up to 200 clients, completing health screenings and identifying physical health risksSupport wellness planning by setting physical health goals and promoting healthy lifestyle changesEducate clients on chronic condition management, appropriate healthcare usage, and health preventionComplete screenings (e.g., metabolic syndrome), medication reconciliations, and health consultationsCollaborate with PCPs, specialists, and community partners to ensure holistic, person-centered careParticipate in interdisciplinary team meetings and provide nursing support across clinic locations as neededRequirements, Skills, Knowledge and Expertise
High School / GED requiredPractical Nursing or Associate of Nursing degree requiredBachelor’s degree in nursing preferredQUALIFICATION(S) : WORK EXPERIENCE / TRAINING / ADDITIONAL REQUIREMENTS
Proficiency in Microsoft Office Suite and Electronic Medical Records preferredA valid driver's license and agency established minimum automobile coverage requiredQUALIFICATION(S) : LICENSURE / CERTIFICATION
