Job Summary :
Responsible for the case management process including assessment / reassessment, service planning, and monitoring of Morton patients.
Essential Duties and Responsibilities :
- Identifies, assesses, and works with high-risk patients
- Conducts initial screenings to determine eligibility
- Develops comprehensive treatment plans
- Communicates with care team
- Engages with patient to assure understanding of goals and assists in meeting goals, monitoring progress
- Coordinates delivery of service
- Leads Transition of Care (TOC) activities
- Refers all identified social drivers of health to Community Health Worker or designee
- Coordinates with interdisciplinary team and documents all services provided
Requirements :
- Current OK nursing (LPN or RN) license in good standing is required.
- 2+ years of demonstrated clinical experience in healthcare.
- 1+ years of demonstrated Clinical Case Management or Care Management experience is required.
- Current BLS / CPR certification required.
- Demonstrated ability to work effectively with staff and the public.
This position has been deemed a Safety-Sensitive position; therefore, Morton reserves the right to refuse to hire an applicant or may terminate an employee in a Safety-Sensitive Position who tests positive for medical marijuana even if he / she has a valid medical marijuana license.
