Job descriptionProvide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures. Responsible to the Clinical Manager. Supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses. Conduct home visits and attend periodic office meetings. Manage an average of 5-8 patients per day, achieving 6-8 productivity points daily. Handle various types of cases including Start of Care, Wound Care, and Follow-up Visits. Complete all patient documentation within 24 hours of each appointment. Participate in on-call rotations, as required. Attend weekly meetings at the clinic and accept schedule assignments. Report to designated work location. Maintain a high standard of patient care while traveling an average of 200 miles weekly within the assigned territory.

- Proficiency in HomeCare HomeBase EMR
- Experience in OASIS assessment within the last 6 months
- Willingness to travel up to 75 miles from the home clinic
