Member Health Assessor

A company is looking for a Member Health Assessor to engage with members and conduct health risk assessments.

Key Responsibilities

Conduct health risk assessments by gathering information from members and stakeholders

Coordinate logistics to support care plan goals and interventions

Maintain documentation according to care management protocols and guidelines

Required Qualifications

High School Diploma or GED is required

Minimum of two years of experience in community settings with at-risk populations is preferred

Proficient in Microsoft Office applications

Experience working with diverse cultures

Ability to work independently and collaboratively in a team environment
