The Position
As part of our expansion, we are seeking dedicated part-time Licensed Clinical Social Workers to provide comprehensive support to individuals and families in their home environments.
This role focuses on serving the adult and geriatric population with a commitment to improving their overall well-being.

Qualifications
To be considered for this role, candidates must :
- Hold an active and unrestricted LCSW license in the state.
- Possess a master's degree in social work from an accredited institution.
- Recent, prior home health, case management and / or discharge planning experience is highly preferred
