POSITION SUMMARY
The Clinical Educator & MDS Nurse plays a key role in supporting high-quality resident care through accurate MDS assessments and meaningful clinical education. This role will serve as a mentor, resource, and leader for the nursing team. The ideal candidate brings strong experience in long-term care, understands the nuances of the MDS process, and demonstrates a passion for developing others.
ESSENTIAL JOB FUNCTIONS
MDS Coordination
- Complete and oversee the MDS assessment process in accordance with state and federal guidelines
- Ensure timely and accurate completion of assessments, care area triggers, and care plans
- Collaborate with the interdisciplinary team to ensure MDS data reflects resident needs and services provided
- Assist with quality measures, reimbursement accuracy, and regulatory compliance
- Provide coaching and support to team members involved in care plan development and documentation
Clinical Education & Staff Development
Assess training needs and develop educational programs to support clinical competency, compliance, and best practicesProvide onboarding, skills validation, continuing education, and mentorship for new and current staffSupport team-building efforts that strengthen communication, collaboration, and accountabilityAssist with performance improvement initiatives and clinical quality programsServe as a resource for policy education, procedure updates, and clinical supportLEADERSHIP & COLLABORATION
Foster a positive, team-focused culture and promote professional growth among staffUtilize strong conflict resolution skills to navigate challenges and support team cohesionServe as a mentor and clinical leader, modeling best practices and encouraging constructive feedbackParticipate in the on-call rotation every other week, alternating with the Director of NursingQUALIFICATIONS
RN preferred; LPN considered based on experiencePrior experience in MDS, care plans, and clinical documentation is requiredExperience as a charge nurse, nurse leader, or clinical educator is strongly preferredBackground in long-term care and / or rehabilitation requiredStrong interpersonal skills, team-building abilities, and a passion for developing othersSolid organizational skills with attention to detail and regulatory accuracyAbility to work collaboratively with nursing, therapy, and interdisciplinary departmentsWHY JOIN OUR TEAM?
