Pathway Hospice, the leading locally owned hospice provider in DFW, is seeking an experienced RN to join our clinical team as a Hospice On Call Registered Nurse.  Our ideal candidate will provide exceptional care and service for Pathway's patients and families.

Responsibilities of this position include the following :

  • Assess patient’s physical condition according to Plan of Care; determine and / or revise plan of care regarding medications and DME
  • Develops, coordinates, and follows the patient's individualized Plan of Care
  • Coordinate with team members / physicians and participate in staff and Interdisciplinary Team meetings
  • Appropriately identify and utilize resources :  Dietary, Pharmacy, Supplies, Home Medical Equipment, etc.
  • Serve as a patient and family advocate to ensure delivery of the highest quality of care, providing education to patients, families and caregivers
  • Complete documentation in a timely manner
  • Communicate effectively with coworkers and patients / family
  • Participate in bereavement support
