• Compensation : $250K base salary
• Bonus : Up to $20K in year one, increasing with partnership
• Partnership Track : No buy-in; typically achieved in months (65% reach partnership within one year; partners earn $300K+)
• Schedule : Monday Friday, 7 : 30 AM 5 : 00 PM (last patient at 4 : 15 PM)
• Patient Volume : Average 18 patients per day
• Panel Size : Max 450 patients
• Model : 100% outpatient, value-based care
• Team Support : Dedicated MA, scheduler, and referral specialist
• Patient Population : Seniors managing 3 5 chronic conditions
• Work / Life Balance : No nights or weekends; built-in administrative time
