Hospice care is typically provided in the patient's home or a specialized facility, focusing on managing pain and symptoms, promoting quality of life, and offering emotional and psychological support.

Role Description

This is a full-time, on-site role located in Fort Worth, TX. The RN Case Manager will be responsible for coordinating and providing quality hospice care to patients. Day-to-day tasks include performing patient assessments, developing care plans, providing direct patient care, and collaborating with interdisciplinary teams to ensure optimal patient outcomes. The RN Case Manager will also provide emotional support and education to patients' families and caregivers.

Qualifications

• Registered Nurse (RN) license in Texas, and in good standing
• Experience in hospice or palliative care is preferred
• Ability to perform patient assessments and develop individualized care plans
• Strong communication and interpersonal skills
• Ability to work effectively in an interdisciplinary team environment
• Valid driver's license and reliable transportation
• Bachelor's degree in Nursing or related field is preferred
• Compassionate and patient-centered approach to care
