Full-Time RN Case Manager - Hospice Nurse (Burleson / Fort Worth)

What You’ll Do :

• Manage a caseload of hospice patients in Burleson and the surrounding area.
• Provide skilled, compassionate nursing care and coordinate individualized care plans.
• Complete routine visits, admissions, recerts, and IDT documentation.
• Collaborate with the interdisciplinary team and participate in biweekly IDT meetings.
• Communicate effectively with patients, families, and providers.
• Maintain accurate, same-day charting with a provided tablet.
• Deliver quality end-of-life care that reflects our values of compassion and respect.
