Description
Summary of Position : The Clinical Pharmacist at Circle the City plays a vital role in providing comprehensive pharmaceutical care to patients, collaborating with a multidisciplinary healthcare team.
This position focuses on optimizing medication therapy, promoting patient education, and ensuring adherence to regulatory requirements.
Responsibilities :
- Collaborate with healthcare providers to review and optimize medication regimens.
- Conduct medication reconciliation to identify and resolve discrepancies.
- Assess appropriateness, effectiveness, and safety of medication therapy.
- Provide direct patient care through medication therapy management (MTM) services.
- Offer medication counseling, ensuring patients understand their prescribed medications, potential side effects, and proper administration.
- Develop and implement vaccine programs, ensuring compliance with federal, state, and local guidelines.
- Coordinate vaccine administration schedules and monitor vaccine inventory.
- Collaborate with public health agencies and community organizations to enhance vaccine access.
- Educate patients on disease management and lifestyle modifications.
- Work closely with physicians, nurses, and other healthcare professionals to develop and implement patient care plans.
- Participate in multidisciplinary rounds and case conferences to discuss patient progress and adjust treatment plans as needed.
- Contribute to quality improvement initiatives related to medication safety and adherence.
- Collaborate in the development and implementation of evidence-based protocols and guidelines.
- Ensure compliance with federal and state regulations governing pharmacy practices.
- Participate in audits and inspections, addressing any identified areas for improvement.
- Stay informed about new medications, treatment guidelines, and emerging healthcare trends.
- Provide educational sessions for healthcare staff on pharmacotherapy updates.
- Serve as an advocate for patients in addressing medication access, affordability, and adherence barriers.
- Collaborate with community resources to enhance patient support.
Qualifications
Qualifications :
- Position requires extended periods of sitting and standing;
- Position requires regular bending and reaching;
- Must be able to lift, carry and push 20 pounds on a regular basis;
- Able to adapt to rapid, unpredictable work flow;
- Able to focus on work at hand with attention to detail;
- Ability to work with difficult patients and those with mental illness.

- Commitment to patient-centered care and cultural competence.
Experience and Education :
- Doctor of Pharmacy (Pharm.D.) degree from an accredited school of pharmacy.
- Active and unrestricted pharmacist license in the state of Arizona.
- Completion of a pharmacy residency program (preferred).
- Experience in an FQHC or primary care setting (preferred).
- Certification in Vaccine Administration (e.g., APhA) is desirable
