Role Description

This is a full-time, on-site role for a Chronic Care Management Director located in New York, NY. The Chronic Care Management Director will oversee the development and implementation of chronic care management programs, coordinate with healthcare providers, and ensure the delivery of high-quality patient care. Daily tasks include managing nursing staff, developing case management protocols, providing training for staff, and integrating critical care medicine practices into patient treatment plans.

Qualifications

• Experience in Nursing and Case Management
• Knowledge in Critical Care Medicine and Medicine
• Capability in Training and staff development
• Strong leadership and organizational skills
• Excellent communication and interpersonal skills
• Ability to work collaboratively with a team
• Master's degree in Nursing, Healthcare Administration, or a related field preferred
• Relevant certifications in Chronic Care Management or related areas are a plus
