Certifications : MSW, LCSW either / or MSW
Must-Have :
- Knowledge of drug / alcohol abuse treatment options
- Experience with patients experiencing homelessness or with mental health histories
- Ability to organize and participate in family meetings
- Familiarity with advance directives and the protective services process
- Skills in care coordination and discharge planning
Description :
This position requires experience in acute medical social work.
The candidate will be responsible for care coordination and discharge planning.
Patient ratios during the day will be 1 : 25.
The position includes participation in interdisciplinary team activities and care conferences.
The applicant should be able to develop and implement a plan of care to meet patient and caregiver needs.
Knowledge of the needs assessment and ordering durable medical equipment (DME) is essential.
The role requires participation in the development of treatment plans.
Weekend coverage will typically be one full weekend every four weeks, subject to departmental needs.
Travel between facilities may be required.
References from supervisors within comparable specialties / settings will be necessary.
The candidate must commit to working three out of five specified holidays.
Flexibility in scheduling is essential to meet the needs of the department.
