Admissions Screener (Hybrid)

Description

We are seeking an experienced healthcare professional to join our admissions team as an Admissions Screener . This hybrid role provides occasional onsite support between our Mt. Lebanon and Canonsburg facilities , with flexibility for remote work when possible.

As the Admissions Screener, you will play a key role in the intake process by reviewing referrals, evaluating clinical documentation, and ensuring appropriate payer source alignment. You’ll collaborate with hospitals, physicians, case managers, and families to ensure smooth transitions while supporting our census goals.

Key Responsibilities

• Manage referrals from initial inquiry through admission, ensuring timely and accurate processing.
• Review hospital records and other clinical documentation to determine admission appropriateness.
• Request and evaluate additional medical or financial information when needed.
• Coordinate with case managers, discharge planners, and families to facilitate seamless admissions.
• Verify insurance coverage in collaboration with the billing department.
• Serve as a trusted point of contact for families, answering questions and providing support.
• Foster strong relationships with referral sources and participate in outreach / marketing activities.
• Support quality assurance initiatives to ensure compliance and satisfaction.

Skills, Knowledge and Expertise

Healthcare admissions or clinical background required (skilled nursing, hospital, rehab, or long-term care strongly preferred).

Preferred Backgrounds :

Previous work as Admissions Director, Hospital Discharge Planner, Case Manager, Social Worker, or LPN.
