Shift : Sunday–Thursday | 8 : 00 AM – 4 : 00 PM
Some on-call required
Location : St. Albans, Vermont
Billing String : St. Albans
Guaranteed Hours : 37 / week
Contract Length : 13 weeks (91 days)

Key Responsibilities :
1. Clinical & Case Management Duties
Acts as case manager for assigned patientsPerforms OASIS documentation accurately and timelyUtilizes evidence-based nursing process for assessments, interventions, and evaluationsCreates and manages realistic plans of care , including patient and caregiver inputMaintains and reviews accurate nursing documentation2. Skills & Equipment
Must be comfortable with high-tech skills :IV therapy, wound VAC, complex wound care, infection control
Familiar with McKesson processes (e.g., visit lock-down, follow-ups)3. Interdisciplinary Coordination
Coordinates care with physicians, providers, therapists, social workers , etc.Supervises LPNs and HCAs, delegating tasks appropriatelyIdentifies additional service needs (PT, OT, SW, etc.)Facilitates safe discharges and transfers4. Compliance & Education
Ensures all documentation follows State, Federal, and agency policiesEducates patients and caregivers (medications, discharge instructions, safety concerns, etc.)Completes all forms like HHABN, Medicare Notice of Non-Coverage in a timely manner5. Administrative & Team Responsibilities
Communicates with schedulers to avoid conflicts in visit typesParticipates in monthly peer reviews and provides / receives feedback professionallyProactively leads case conferences with team members and managersAdvocates for high-quality, patient-sensitive careQualifications Required :
Active Vermont RN License (in good standing)Strong knowledge and hands-on experience with OASIS documentationExperience in home health nursing , case management, and patient educationUp-to-date knowledge of state and federal home health regulationsAbility to work independently and manage a caseloadQualifications Required :
