SUMMARY : In this vital role, you will provide high-quality, individualized patient care in collaboration with our nursing staff. Your proactive approach and commitment to outstanding patient service will contribute significantly to our team’s success.

RESPONSIBILITIES :

• Patient Care & Service : Assist residents with activities based on personalized care plans; facilitate transportation to and from activities; provide support for leisure activities, and offer emotional support to residents and their families.
• Safety : Help maintain a clean and safe environment for residents.
• Communication & Documentation : Communicate effectively with families, direct them to appropriate staff, respond promptly to resident needs, and relay important information during team briefings.

PHYSICAL REQUIREMENTS :

This position requires medium work, which involves exerting 20 to 50 pounds of force occasionally, and / or 10 to 25 pounds of force frequently, and / or up to 10 pounds of force constantly. Frequent walking, standing, or squatting is required.

The stated base pay range reflects current potential earnings for a successful full-time candidate and may change in the future, with opportunities for additional compensation. Salary is determined by experience, qualifications, specialty, location, and other relevant factors.
