Job description

POSITION SUMMARY : Perform the duties required to promote healthy lifestyle changes in clients with chronic conditions, through education on the disease process, exercise and nutrition, A1C Screening, Cholesterol Screening. Instruct clients in clinical appointments, also in their home to facilitate the learning process in a more relaxed setting.

POSITION QUALIFICATION REQUIREMENTS

Education : High school graduate or equivalent. Medical Assistant preferred.

Experience : Minimum of one (1) year experience as a Medical Assistant and or Community Health Worker. Some general office experience required.

Skills and Abilities which may be representative but not all inclusive of those commonly associated with this position :

  • Ability to implement plan of care as devised by MD and RN.
  • Strong decision making and prioritization skills.
  • Ability to work with minimal supervision.
  • Requires good written communication and organizational skills.
  • Communicates effectively with health center team and clients.
  • Exhibits teamwork mentality.
  • Good computer skills including the use of word processing and e-mail software.

Transportation Requirement : Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri.

License(s) / Certification(s) Required : CPR certification

FUNCTIONS OF THE JOB

Essential Functions

  • Schedules and visits clients diagnosed with a chronic disease to assess their environment, needs, and available support. Completes accurate documentation and provides follow up information to the provider regarding the visit.
  • Provides clients with educational materials and answers questions.
  • Schedules provider appointments; makes emergency home visits to monitor patients.
  • Responsible for filing appropriate documentation in client files; monitors and maintains patient tracking system.
  • Assist with health and wellness initiatives.
  • Recruit patients for, schedule, and facilitate health education groups.
  • Facilitate and assist with (chronic disease) community referrals.
  • Assist with facilitation of referrals for high-risk patients.
  • Assist with implementation of programs / initiatives.
  • Other duties as assigned.
  • Marginal Functions

  • Participate in and Plan community health fairs events (Required)
  • Maintains accurate travel log.
  • Supervises : None
