We are looking for a Full-time (Mon-Fri, 8 : 30am-5pm) Nurse Practitioner (NP) for a home health position in a PACE setting. Program of All-Inclusive Care for the Elderly (PACE) is committed to keeping Hudson County seniors healthy, safe, and living at home. Our innovative program addresses the needs of elderly individuals who qualify for nursing home level of care but helps them remain in their own community surrounded by family and friends. The Nurse Practitioner must have at least 1 year of experience working with the frail or elderly population.
