With integrity at the core of our mission, you will bring exceptional support to both patients and your local community.\n\nAs a Registered Nurse Case Management Specialist (RNCM), your responsibilities will include : \n
- Conducting in-person patient interviews and comprehensive physical assessments.\n
- Overseeing the development and ongoing management of individualized care plans, coordinating with home health aides, LPNs, RNs, and other professionals.\n
- Communicating patient conditions effectively and collaborating with healthcare providers to adapt care as patient needs change.\n
- Educating patients and their families on essential home health care techniques, such as wound care, IV administration, and medication management.\n
- Focusing on reducing readmissions through proactive care and promoting continuity of care.\n\nTo qualify as a Registered Nurse Case Management Specialist (RNCM) with us, you will need : \n
- A current and unrestricted license to practice as a Registered Nurse (RN).\n
- Current CPR / AED / BLS / First Aid certification.\n
- Reliable transportation to and from care locations.\n
- At least one year of professional RN experience in home health or a related field; experience in case management or utilization review is preferred.\n
- OASIS experience is preferred.\n
