Registered Nurse Case Manager

Hospice Registered Nurse Case Manager\n\nSalary : $80-89k / yr BOE\n\nSchedule : M-F| 8 : 00AM-5 : 00PM\n\nLocation : Hilliard, Ohio, and surrounding areas\n\nRequirements of the RNCM role : \n

• Licensed Registered Nurse\n
• BLS Certified\n
• Valid driver's License and Auto insurance Coverage\n
• Reliable transportation\n
• A minimum of 1 year of Clinical experience\n
• Hospice experience preferred\n\nAbout the RNCM role : \n
• Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met.\n
• Perform evaluations for hospice care along with informational visits as needed.\n
• Consult with the attending physician and hospice medical director concerning changes in patient condition requiring order changes and / or changes in the plan of care.\n
• Submit complete, accurate, and relevant clinical notes promptly.\n
• Participate in IDG and case conferences\n
• Performs on-call duties per policy and need\n
• Conducts an initial and ongoing comprehensive assessment of the patients\n
• Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care.\n
• Supervise and evaluate the care given by the hospice Aide at a minimum of once every 14 days or as needed.\n
• Provide care plan to Hospice Aide and LPN as part of the IDG team.\n
