IHBT Counselor

At Appleseed Community Mental Health Center, Inc., we are dedicated to helping families and individuals lead healthy, fulfilling lives. We offer comprehensive services in behavioral health, housing, outreach & prevention, substance use disorder, and victim support. We are currently seeking a passionate and skilled Intensive Home-Based Treatment Counselor / Social Worker (IHBT) to join our team.

Role Overview :

As an IHBT Counselor, you will provide vital support to youth and families directly in their homes and communities. Your role will involve managing a small caseload, offering crisis intervention, and applying trauma-informed care to help clients navigate their challenges.

Key Responsibilities :

• Deliver home-based therapy and support to a caseload of 4-6 youth / families.
• Provide flexible, on-call crisis response and face-to-face support as needed.
• Assess and address safety concerns, risk behaviors, and develop personalized safety plans.
• Apply comprehensive trauma-informed mental health treatments and best practices.
• Engage in individual and group supervision, complete core training, and participate in ongoing professional development.
• Collect feedback to ensure high-quality service and contribute to a supportive team environment.
