Job descriptionMonday through Friday, 8 : 30 AM – 5 : 00 PM Occasional weekend shifts may be required Cantonese Mandarin Armenian Russian Tagalog Vietnamese Conduct in-home visits to assess client needs and coordinate support services Develop and monitor individualized service plans Maintain accurate documentation and case notes Collaborate with families, service providers, and internal teams Ensure clients are receiving appropriate services and supports in line with program goals Experience working with individuals with intellectual or developmental disabilities preferred Strong communication and organizational skills Ability to manage a caseload independently Valid driver’s license and reliable transportation Proficient in documentation and basic computer use

Service Area : In-home visits across the Burbank area, supporting adults with intellectual disabilities
Compensation :
- $26.00 to $29.00 per hour, depending on experience
- Mileage reimbursement for travel between client homes
We are hiring bilingual Case Managers to join our team and provide personalized, in-home support to adults with intellectual disabilities. This role is ideal for someone who is compassionate, reliable, and comfortable working independently in the community.
