Rural Health Transformation Program Manager, Remote Patient Monitoring

The Virginia Hospital and Healthcare Association is seeking a Rural Health Transformation Program Manager, Remote Patient Monitoring to lead a statewide initiative expanding access to innovative, continuous care technologies for rural communities across Virginia.

This role within VHHA Foundation manages a multi-partner pilot program funded through Virginia’s Rural Health Transformation Program , bringing together hospitals, health systems, vendors, and state agencies to improve care quality, coordination, and long-term patient outcomes.

Key Responsibilities

• Serve as project lead for the Rural Remote Patient Monitoring Initiative and primary point of contact for partners and stakeholders
• Manage vendor selection, contracts, and implementation of continuous care technologies
• Support hospitals and pilot participants with implementation, technical assistance, and workflow integration
• Track program performance, outcomes, and data in collaboration with analytics and grant operations teams
• Ensure compliance with grant objectives, reporting requirements, and funding timelines
• Explore alternative payment models and reimbursement strategies to support long-term sustainability
• Build strong relationships with health systems, vendors, clinics, and state agencies
• Contribute to policy and legislative discussions related to rural health transformation
• Assist hospitals in identifying and pursuing additional funding opportunities
