California Licensed Practical Nurse

A company is looking for a Licensed Practical Nurse (CA license required).

Key Responsibilities

Support delivery of health care to patients by performing activities under the direction of Nurse Practitioners

Provide one-on-one coaching and support to patients managing chronic conditions

Conduct comprehensive assessments of patients' health status and help execute personalized care plans

Required Qualifications

Active multi-state compact Practical Nurse License

Active California State LPN / LVN license

5+ years of clinical experience as a Licensed Practical Nurse

Experience in patient education for individuals managing chronic conditions

Prior experience in a high-growth, fast-paced startup environment is a plus
