Job descriptionHigh School diploma required. Associate degree in human services preferred. Minimum of two years' experience in long-term care. CPR certification required.
Duties :
- Collaborate with patients / residents and their families to provide support and resource information.
- Respond to issues identified by patients / residents and their families.
- Facilitate patient / resident transfer within the nursing center to ensure a seamless transition and adjustment.
- Document patient / resident transfers and adjustments accordingly.
- Participate in the health care decision-making process under the direction of the Social Services Director
- Assist residents to and from needed outside physician appointments.
