About The Position

Job Overview :

We are seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to join our Behavioral Health team. This role is vital in supporting patients and their families by addressing social, emotional, and environmental needs through assessment, counseling, crisis intervention, and coordination of care.

Key Responsibilities :

Conduct psychosocial assessments to identify patient needs and develop individualized care plans

Provide counseling and crisis intervention services to patients and families

Collaborate with interdisciplinary teams to develop discharge plans and connect patients with community resources

Document all services and communications in compliance with facility, state, and federal standards

Advocate for patients’ access to needed services and ensure continuity of care

Participate in case conferences, rounds, and staff meetings as required

Ensure patient confidentiality and maintain ethical standards of practice

Job Requirements

Qualifications :

Minimum 1 year of recent LMSW experience in a clinical, hospital, or behavioral health setting

Valid LMSW license issued by NYSED (New York State Education Department) – Primary Source Verification required

Master’s degree in Social Work from an accredited institution

Knowledge of psychosocial issues impacting patient care and behavioral health

Excellent communication, assessment, and documentation skills
