As a Case Manager, you will be responsible for assessing the healthcare needs of our clients, developing individualized care plans, coordinating services with other healthcare providers, and advocating for the best possible outcomes for our VA clients.
You will work closely with our interdisciplinary team to ensure that our clients receive the highest quality of care and support.

Common skilled services :
- Wound care (burns, pressure sore, wound vacs etc.)
- Med box fill
- Med administration (insulin, filgrastim, growth hormones,)
- Infusions- IVIG, NS
- Physical Assessments
- Catheter exchange (indwelling / suprapubic)
- Ostomy care
- Bowel program
Expectations and Duties :
- Run a care note exemption audit and note any care note exemptions
- Update the client care database with recert dates, comps, and / or other important notes
- Complete the Client Care Meeting Report and send it in to the DON Attend and participate in Weekly Client Care Meeting at main office
- Perform handwashing observations and submit form Perform bathing observations and submit form
- QA National account client documentation
- Create Care Plans
- Admissions
- Weekly On Call expectation (4 days out of the month)Duties :
- Perform in-person client supervisory visits every 14 days, if applicable
- Perform CNA and Client supervisory visits in person every 60 days
- Complete skilled assessment, client supervisory visit, and update care plans during visit
- Print out copies of updated care plans and leave them in the client’s home
- Perform PRN care plan change visit and complete documentation
- Manage MD orders and medications
- Fax new orders and updated skilled care plans to MD
- Leave a hard copy of care plans with every change within 7 days of change
- Communicate with MD for client status changes
- Perform RN skilled service visits (catheter changes, wound care, medication box refills) as needed
- Coordinate care with other agencies as needed
- Manage client complaints
- Provide constructive feedback to CNA’s and SN’s providing care
