Position Overview :

We are looking for a dynamic and autonomous Nurse Practitioner to take on the role of Program Manager for our Wound Care Program.

The successful candidate will play a crucial role in overseeing and managing the program, providing direct patient care, developing chronic care plans, and collaborating with referring providers to optimize patient outcomes.

Responsibilities :

• Clinical Leadership :
• Provide direct patient care through all available channels and products.
• Conduct comprehensive assessments, develop treatment plans, and implement evidence-based care practices.
• Monitor and evaluate patient progress, adjusting treatment plans as needed.
• Program Management :
• Oversee the day-to-day operations of the Wound Care Program.
• Collaborate with interdisciplinary teams to ensure seamless patient care coordination.
• Develop and implement protocols, policies, and procedures to enhance program efficiency and effectiveness.
• Chronic Care Planning :
• Develop personalized chronic care plans for patients with wounds, focusing on long-term management and prevention of complications.
• Educate patients and families on self-management strategies and lifestyle modifications.
• Communication and Collaboration :
• Establish and maintain effective communication channels with referring providers to ensure continuity of care.
• Collaborate with healthcare professionals, including physicians, nurses, and other specialists, to optimize patient care.

Qualifications :

• Current and unrestricted Nurse Practitioner license in AZ.
• Certification as a Nurse Practitioner with a specialization in wound care management preferred.
• Minimum of 1 year of experience in wound care.
• Strong interpersonal and communication skills.
