Job descriptionProvides, coordinates, and directs home nursing care according to physicians' orders and agency policies. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical / Vocational Nurses. Required to conduct home visits and periodic office meetings. Must have reliable transportation with a current driver's license and agency-required liability insurance. Average patient visits range from 5-8 per day. Contractors will be assigned 5-10 patients daily, equating to approximately 6-8 points. Required to reach 30 points weekly. Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. Proficient in completing documentation within 24 hours of an appointment. Must be open to on-call rotations, although specifics may vary by location. Travel up to 75 miles from the home clinic is required. Mileage is reimbursed at the standard rate. Responsible for covering specific territories based on demand. Dress code is scrubs, and all professionals must be proficient in electronic medical records, specifically HCHB pointcare. Local candidates accepted at the same rate, but must meet all other requirements.
