Behavioral Health Care Manager

A company is looking for a Behavioral Health Care Manager (BHCM) - Remote.

Key Responsibilities :

Conduct initial clinical member biopsychosocial intake and administer behavioral health assessments

Engage in follow-up visits and brief interventions, acting as a liaison between specialists and the care team

Lead weekly interdisciplinary team meetings and document member progress in a HIPAA-compliant system

Required Qualifications, Training, and Education :

Bachelor's Degree required in nursing, social work, or related health and human services disciplines

Experience as a Certified Case Manager (CCM), Community Health Worker (CHW), or Peer Support Specialist (PSS) preferred

At least one year of clinical care management experience, preferably with cancer or advanced illness patients
