Service Area - Muncie, Alexandria, and surrounding areas
Monitor carefully the patient status and progress toward goals, as well as compliance with physician's orders. Responsible for educating patients and caregivers, while preparing the patient / caregiver for discharge as appropriate.
Provide skilled speech language pathology in accordance with the physician's plan of treatment.
Observe, record, and report the client's reaction to treatment and any changes in the client's condition to the physician, DON / Clinical Manager, according to policy.
Develop, implement and revise the client's plan of care.
Promote coordination of care through interdisciplinary case conferences and communication with physician and field staff.
Perform therapy interventions utilizing standard speech language pathology techniques and skills as appropriate for the condition of the patient.
Previous Home Health experience is preferred but not required.
Must be currently licensed to practice in the state of Indiana
Must be CPR certified
Current Drivers License, vehicle insurance and access to a dependable vehicle
